Positive behaviour support

Behaviour support plans that help on a hard day.

Write the plan with your PBS practitioner, sign it off, and give carers clear steps in green, amber and red. Every behaviour record then shows you whether the plan is working.

Free for 30 days. Positive behaviour support is part of your plan.

Green, amber and red. The right step at the right time.

  • GreenWhat keeps the person well and settled every day.
  • AmberThe early signs, and how to help before things escalate.
  • RedThe least restrictive way to keep everyone safe.
  • AfterSupport for the person and the staff once it is over.
The green, amber and red sections of a PBS plan, each with clear steps for staff
  • The person firstHow they communicate, what they enjoy, their routines and sensory needs open the plan.
  • Understanding behaviourWhat each behaviour looks like, its likely reason and what tends to trigger it.
  • Capacity and consentCapacity, best interests, advocacy and who helped write the plan, all recorded.

Written with your practitioner. Signed off by you.

External PBS practitioners get their own login for the people they work with. They draft the plan and see the behaviour records. A manager signs off every version.

  • Locked when signed

Signing off locks the version. Any change starts a new draft, so the plan in use never shifts under anyone.

  • Every version kept

See each version, who signed it off, when, and a note of what changed.

  • Read and confirmed

Carers confirm they have read each new version. You can see who has and who has not.

A carer recording behaviour on a phone: date, time, behaviour, where it happened and what came just before

Record behaviour in the moment, on a phone.

Carers write what happened before, during and after, in plain words. It takes a couple of minutes.

  • The highest stage reached, how intense it was and how long it lasted.
  • Whether the plan's strategies helped.
  • Any restriction used, and whether it was in the plan.
  • Record behaviour sits on every shift card, and in care logs when Incident or safeguarding is picked.

Nothing slips through. A restriction that is not in the plan, or any injury, flags an urgent review and a debrief for a manager or DSL.

See whether the plan is working.

Every behaviour record feeds the analysis. Look back over 28 days, 90 days or 6 months.

PBS analysis showing records per week falling over 12 weeks, with breakdowns by behaviour, time of day, place and whether strategies helped
  • Records per week over the last 12 weeks, so you can see the trend at a glance.
  • Broken down by behaviour, time of day and where it happened.
  • How often the strategies helped, from the carers who were there.

Your own records, nothing more. The figures come only from behaviour records in Care Pathway Pro. Use them alongside your practitioner's assessment when you review the plan.

Restrictive practices, recorded and reviewed.

Every restriction the plan allows sits in one register, with the reason, the legal basis and a review date.

  • Record the legal basis: consent, a best interests decision, a DoLS authorisation or a Court of Protection order.
  • A six-month chart of restrictions used, with any not in the plan shown in red.
  • Restrictions only change through the plan, so every change is signed off.
The restrictive practices register with type, reason, legal basis and review date, above a six-month chart
An easy read summary of the plan, shared with family

Families see what helps. Not the incident log.

Share a short, easy read summary of the plan with the family, so everyone supports the person the same way.

  • Written in plain, short sentences.
  • A manager shares it. The family reads it in its own tab of the care plan in Family Connect.
  • The full plan and behaviour records are never shared.

  • Staff who support someone with a PBS plan need PBS awareness training. Untrained staff cannot be put on that person's shift.
  • Download the signed-off plan as a PDF whenever you need it.
  • Every plan version and behaviour record can go into the person's records pack for inspection. See CQC evidence
